children’s mental health Targeted Case Management
MHR’s Children’s Targeted Case Management teams provide support to children who have been diagnosed with Severe Emotional Disturbance and their families. MHR provides person-centered, personalized services for youth who may have co-occurring medical, social, and substance use challenges.
Our TCM case managers help people gain access to services needed to meet their mental health needs, achieve recovery, and live independently in the community. Services are provided by skilled mental health practitioners who are supported by a team leader and a clinical supervisor. Case managers work closely with each family to create a treatment plan, based on assessment of functioning, needs and strengths. We meet with youth and their families, sometimes separately, in their home, school, or in the community. We meet 1-2 times a month on average, but sometimes more often if required to get the youth proper support.
We coordinate services with other providers to help each family achieve personalized goals on their individual treatment plan. Services include:
Assessment of functioning and needs.
Assessment of strengths and support systems.
Referrals to medical, psychiatric, dental vocational/educational, social and benefit services.
Connection to county-based services as well as referral to state-operated services for higher levels of care, if needed.
Coordination and ongoing monitoring of services.
Crisis planning.
Advocating with schools and taking part in IEP/504 plan meetings.
Access to MHR's health and wellness programming including tobacco reduction and comprehensive dental support.
